I only have had it happen in D when the car shifted to second at low speed then I went wot and it shifted back to first, then hit the rev limiter. Never happened when wot from slight rolling start (hasn't yet shifted to second).

Had the flash, still did it a couple of times, but lately has been shifting wonderfully.

I have about 2800 miles on the car.
