There is a tiny notch on one side of the plastic plate over the opening. Insert a very tiny screwdriver or a paper clip to pry the plate up so that it pops out.

After you eject the disc, clean it carefully with a very fine microfibre (like the kind used for eyeglases).

Re-inset disc and pop the face plate back in. That should take care of the disc error.
